Health  Screening for In-Person Services and Programs

In order to attend, usher, lead, or staff any in-person service or program at Kane Street Synagogue, those who will be present on the premises affirm the following statements, no more than 24 hours in advance of the start of any service:


In the past 10 days, I/we have not experienced the start of any symptoms of COVID-19, including a fever of 100ºF or greater, a new cough, new loss of taste or smell, or shortness of breath.


In the past 10 days, I/we have not received a lab-confirmed positive result from a COVID-19 diagnostic test (not a blood test) that was my first positive result OR was AFTER 90 days from my previous diagnosis date.
[Please note that 10 days is measured from the day you were tested, not from the day when you got the test result.]


If attending an indoor event, I/we am/are considered fully vaccinated against COVID-19 by CDC guidelines, I/we am/are considered fully vaccinated against COVID-19 by CDC guidelines, and, if applicable, may be accompanied by member/s of my/our household under the age of 18 who are unvaccinated or partially vaccinated..
[Please note that to be considered fully vaccinated by CDC guidelines, two weeks must have passed since you received the second dose in a two-dose series (Pfizer or Moderna) or two weeks must have passed since you received a single-dose vaccine (Johnson & Johnson).]


To the best of my knowledge, in the past 10 days, I/we have not been in close contact (within 6 feet for at least 10 minutes over a 24-hour period) with anyone who is currently diagnosed with COVID-19 or who has been told they have symptoms of COVID-19.
[Healthcare personnel and clinical staff who were in appropriate PPE are not considered close contacts in these scenarios.]


In the past 10 days, I/we have not returned from an international destination.
